Lack of medical necessity is the #1 reason CMS denies E/M claims. In fact, the Agency reports erroneously paying $4.2 Billion for E/M claims, and recently announced that they are coming after it in 2015.
You may think that you don’t have to worry if your medical necessity documentation leads to lower E/M levels, but you’re mistaken. Not only are you losing revenue that is rightfully yours, but audits are on the lookout for E/M levels that fall outside of CMS norms (higher or lower).
So, what can you do?
