Changes to Waived Tests Approved By FDA

The Centers for Medicare & Medicaid Services (CMS) requests that Medicare Administrative Contractors (MACs) change modifiers and otherwise accommodate changes to waived tests approved by the Food and Drug Administration under the Clinical Laboratory Improvement Amendments of 1988 (CLIA). Contractors should use a QW modifier to identify covered tests.
